What to Bring

So you called our office, spoke with our team, and have an appointment date. Congratulations on your first step to better mental health! To make your first visit run smoothly, here’s what you should bring with you to your appointment:

  • Current ID

  • Insurance card or information

  • Your referral, if you have one

  • List of medications you’re currently taking

  • Any relevant medical information

  • Questions to ask your provider

Try to arrive 10 minutes early to give yourself time to fill out any paperwork.
